Step 1: Identify Competencies
Competencies are groups of behaviors that comprise the necessary activities of a job. For example:- Communication - Speaking, Listening, Writing, Presenting
- Decision Making - Interpreting data, Good Judgement, Mitigating Risk
- Leadership - Supervising others, Leading teams, Leading department/division/organization
- Interpersonal Skills - Honesty, Tactfulness, Empathy
Step 2: Select Items For Each Competency
Selecting items can be challenging when there are hundreds of potential items for each competency.
Step 3: Determine the Rating Scale to be Used
The rating scale is the set of alternative responses that are given to participants to rate each item on the survey. Rating scales are usually 4 or 5 points.

Your feedback is an important part of our performance management program. This tool is intended to gather broad feedback in the core competencies
and role responsibilities that are important for the on-going success of our organization.
In an ongoing effort to reinforce job performance fundamentals, you are asked to respond to the statements that follow.
Please read each statement carefully, refer to the rating scale provided, and decide to what extent you engage in the behavior described.
You will use a 5-point rating scale, from 'Never' to 'Always'.
